Looking for a plugin

Hello,

I am looking for a plugin/script that does the following:
On the main page, there should be a simple submission form with 3 fields (wish, name, email). Once the visitor clicked the submit button, the content of those 3 fields should be displayed right under the form along with all of the content from previous visitors?

Any ideas on how to realize that? For the form, I could just use any form plugin, but I don’t know how to display all the entries…
