unsuccessful checkout/cart

Hello, I seem to be having an issue with customers trying to purchase/checkout on my site. Earlier this month a customer contacted me and said that anytime they tried to add to cart it would be empty or redirect them to the home page. Since other customers checked out fine i assumed maybe they were doing something wrong but since then a few other customers had the same issue and probably a lot more that didn’t contact us after some time the issue went away and they could checkout fine. Im not sure whats causing this and anytime i test the cart/checkout myself everything works fine. Below is the status report
