First/Last Name placeholders no longer in same row

    Hello,

    When I first created the form viewed at the bottom of the page linked, the first and last name fields would appear in the same row. Somewhere along the way it seems the behavior has changed and they stack now. Please advise what I can do to get them to align in a row again?
