Is there any form plugins that allow me to create complex forms?

    i need a form on my page that starts with one question which has multiple answers. the user selects one answer, and depending on which answer is selected, a different set of questions is displayed each time. the user then fills in their set of questions and presses ‘submit’.

    i kno i could probably create such a thing in Dreamweaver, but i hope theres plugin out there that can let me do this.
