Feature request: don’t ask this question for existing customers

Thanks for this handy little plugin! Feature request: I have customers that come back again and again to make purchases. I feel a little silly asking them this question over and over, even if they already answered once before.

What if instead of having a checkbox for Enable/Disable, you had three radio buttons:

( ) Disable
( ) Enable for all customers
(x) Only enable for customers who haven’t already answered this question
