The text in the Estatik Request form can be changed via the widget menu.
Appearance>>Widgets>>Widget Estatik Request Form

As for the privacy checkbox – its change is possible via code modification only. It can be overridden in the child theme in the folder your-child-theme/estatik/widgets/es-request-widget.php
