Can’t see the form

    I had added Autooptimize, Fast Velocity and a3 lazyload to my website, and the form is no longer displaying. I have disabled those plugins, but the issue remains

    I’ve visited your web page and there is not included any form on it. I see you are displaying a cached version of your website. Please, indicate exactly the URL to the page where the form is inserted, and be sure to purge the website’s cache first.

    It should appear after VOICE OVER CALCULATOR

    the shortcode is [CP_CALCULATED_FIELDS id=”8″]

    When I look at the page code, it is’t on there. When I disable the plugin, the shortcode appears.

    cache etc all cleared.
