I hope you’re well today and I’m sorry for the delayed response!

Usually issues like this one are related to some CSS (from theme or some other plugin) that “overrides” the Forminator theme and breaks it.

I’ve visited the page in question and the form looks fine there now so I guess you managed to solve that meanwhile, right?

If you still need some assistance with the issue, let me know, please where on your site I can see such broken form “live” and I’ll be happy to help.

unfortunately I still haven′t solved it

you will see the broken form, I realized after it′s being refreshed it looks alright..

I’ve tested a preview on my end and can’t replicate the same. On the front end of your site, all is well. Please see this screenshot:

If you still see this issue on your end, would you please clear browser cache? Please view if the form is showing the same in different browsers and let us know in which browser this form is not showing well for you.

I have used google chrome and safari and on both it shows broken, looks good only after I refresh the page (as in your screenshot)
On the phone only date box looks broken.
I have cleared the browser cache..

Sadly when I tried replicating the issue on my end, I was not able to replicate it on my end. I went ahead and also checking multiple locations, devices and browsers to be double sure and I noticed that the form loaded like a charm. Seems like some issue with the cache or the ISP on your end. Can you try multiple networks and devices to check this as we were not able to replicate the same on our end? Please let me know how that goes for you. I am looking forward to hearing from you on it.

Hello, that all looks good I also get that but ONLY after I refresh the page.I still have the problem calandar on the phone looks bad and whole form desktop version doesn′t look good. On some mobile versions it′s not visible at all..

I was able to see the issue on your site but it doesn’t seem to happen on all browsers and on each load.

Could you perform a full conflict test to see if there’s anything conflicting with Forminator on your site?
You can do this by disabling all other plugins and switching to the default WP theme and see if the form loads properly after that.
If it does, enable them back one by one until the issue happens again, that should tell us where the conflict is so we can debug it further.

I suggest doing this test on a staging site or if you don’t have one make sure to have a backup of your site ready before doing the test.
