Trying to link contact form to a url

Currently having problems linking my contact form. I cannot get the form to appear on my website. I have my website aphflest.com when you click on the about/contact page if you scroll to the bottom I have at the bottom portion you can contact me on this page. This page was hyperlinked to where when you clicked it. It would go to the contact form where you could send me a message. But I accidentally deleted it. Now I need help getting the contact form linked to the hyperlink again.
