Checkboxes Don’t Display, Dropdown Doesn’t Drop Down

My form displays the NAMES of the checkbox items, but does not display the checkboxes themselves, so the user cannot make selections.

Also, when I insert a drop-down menu, all the items appear in a vertical list, not a drop-down, and the user can only make multiple selections by holding down CTRL or Command (I have checked “multiple” in the form setup).

I’m on a recent version of WordPress (can’t find the exact version, but it’s less than a year old) with Customizr theme. This occurs in the latest versions of Chrome and Firefox.
