Duplicate fields inserted into form

    I have inserted a form for user registration. Using the ACF Field Group, I have a group of fields that have conditional logic to show only if a field is true or false.
    This works, but the non conditional fields are being displayed twice.

    The site is set to show only if logged in, so PM me for login details.
