Logout Fails

Whenever anyone tries to log out using the Log Out button on my Navigation bar it gives an error and redirects you back to the site. When I look at my Header Php file it says this as the function of clicking that button:

• /wp-login.php?action=logout&redirect_to=<?php echo urlencode($_SERVER[‘REQUEST_URI’]) ?>”>Logout

I have tried altering this a bit, but nothing seems to work. Any suggestions on how I can modify this so when you click it you are logged out and redirected to the Home page?
