Labels OR Templates?

Hi, I am very new to wordpress and am trying to figure out the best way to set up my website. I don’t know if it would be best to use labels or different templates or Both…

Quick Overview: I want users to submit Reviews, Projects, etc on the website. I have 2 categories set up: Reviews and Projects.

• Now, I would like seperate forms for them to fill out with relevant questions weather they are submitting a Review OR Project.
• I would also like to show a series of questions depending on the form (Review, Project) they are filling out Which will show as TAGS.
  Ex. For Review I would like to ask: Recommended by: Friend, TV, Internet
  AND Also Ask Type: Movie, TV Series, Book, etc.

  For Project I would like to ask: Kids, Outdoor, Indoor, Game, etc.

What would be the best way to accomplish this? Does anyone have any suggestions on implementing this? Thank you for your comments.
