Please remove forced input fields

I need to create a form with only 2 fields.

I don’t need to ask for first name, last name, email address, subject and description.

I know I can change the title of these but it’s still 5 fields forced on to a form where I only need 2 fields.

Please can these be removed as mandatory? Or at least some of them?

I can already see which of my members opened the ticket, so I really only need 1 field actually.
