Search widget request (will pay)

Hi,

I need a custom search widget that will let the user choose wich of about 5 categories that should be included in the search. The users defines this with checkboxes below the search field. Unchecked categories are not included. Checked ones are.

My site has around 15 categories and I will need to be able define wich categories should be available to the user – below the custom search box.

I can’t find anything like this anywhere. If you can make such a widget for me I would be happy to pay for your time spent – depending on how much of cause!
