SIMPLE contact form (set from admin, use with template file)

Ok I’d need a VERY VERY SIMPLE contact form (for a client with no php/html knowhow) which could be then inserted in to page template. NOTE not from the admin to page content, the client will eventually mess it up. So to be used with theme page template please. THANKS for your help. I’ve used a good 24hrs and still haven’t came up with a solution to this…
