Easy to Use, Great Support

I tried several similar plugins like Form Maker with mixed results. Form Maker gives me the features I need in an easy to use platform. Add-ons are reasonably priced. The customer service is also excellent. The only reason I did not give them 5 stars is that they only give you 7 questions, including email and name, per survey in the free version. It limits what you can do.
