Enter Number to direct to URL

Hi all, new to the forums.

Have a question with a simple fix – Been on the forums and google for about an hour, but probably just don’t know the terms, so I’m finally giving in. I would like an input box that would perform the following:

Please enter your number: XXXX (lets say that XXXX = 1234)

When clicking the button (lets say the ‘find my number’ button), I would like the page to open to:

https://www.mycrappywebsite.com/directory1/directory2/1234/

Alternatively, I would like an input of ‘4321’ to open to:
https://www.mycrappywebsite.com/directory1/directory2/4321/

Make sense? Thanks for the help guys.
