Contacts Not Adding To Account

I’ve looked through a number of topics that seem to be running into a similar issue but can’t yet find a resolution.

When adding an e-mail address to my form I am getting a success message and e-mail alert but nothing is being passed into Constant Contact. I’ve turned on debugging but don’t have any messages and can’t see any obvious additional fields that don’t look like they have been added by this plug-in.

I need to know what else I can test and check and any additional debugging I can do.
