Recommendation for simple contact form solution

Hi,

On the site I’m creating now with Sirius Lite, I have the need for a super simple solution where visitors who want to contact us, need to fill in a super short form, like:

Name:
Message:
Mobile:
E-mail:

Right now, I need 9 different forms, and the message should be redirected to an e-mail address.

I can create 9 sub pages, but it would be best to just have a short intro text on a contact page, and under that – a form where you choose which “function/person” to contact and then the 4 fields are mandatory.

Do you have a simple solution that doesn’t require me to code this?
