Contact Form 7 integration broken

    The below code used to work fine for all of the registration/signup forms on our site. Recently the below is what shows to the general public. The form still works but not the checkbox for them to join our newsletter. Any fix for this?

    In my form I use list:3 and that works fine !
    You can add more lists with list:3 list:4 etc
