How can I show thumbnail in caption, large image in post?

I realize the newest version of Worpress has this option, but I was wondering if anyone knew a quick way to show the large or medium size image in the full post, but with the caption of the story having the thumbnail.

I would also like to have this change affect older posts. Is it possible?
