Installation

1. Upload eyewear_prescription_form.zip to the /wp-content/plugins/ directory
2. Activate the plugin through the ‘Plugins’ menu in WordPress
3. You will see EPF option in admin panel.
4. Under EPF option: You can manage Forms,Steps,Options, prescription fields, settings etc.

I think this is a great plugin, powerful and rich in content. It enables prescription forms on the page and lets customers enter or upload ophthalmic prescriptions. And glasses can be customized with a variety of glass options, such as coatings, lens types, materials…
